Customer qualification for the offer is at CenturyLink’s discretion. Customer will receive an email or postcard with redemption instructions within 3-4 weeks from enrollment and will be required to complete online redemption form within 60 days of receipt. Customer must pay first bill in full, with account current and in good standing. The Card will be sent 4-6 weeks after installation and confirmation that all eligibility and retention requirements have been met. One Card per customer.
